Service Description

Care Bridge Medical, Inc. ("Care Bridge," "we," "us," or "our") provides a platform for physician-led, AI-assisted medical second opinion consultations. Our service connects patients with US-licensed specialist physicians for independent review of medical records, video consultations, and signed second opinion reports.

Care Bridge provides advisory second opinions only. Our services are not a substitute for the care provided by your local treating physician, and do not establish an ongoing physician-patient treatment relationship. All final medical decisions remain with you and your treating physician.

Not an Emergency Service

Care Bridge is not an emergency medical service. Do not use Care Bridge if you are experiencing a medical emergency. If you are experiencing symptoms that may indicate a medical emergency, call your local emergency services immediately: 119 (Taiwan), 999 (Hong Kong), 995 (Singapore), or 911 (United States).

Eligibility and Account Responsibilities

To use Care Bridge, you must be at least 18 years of age and located in a jurisdiction where our services are available. You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account.

You agree to provide accurate, current, and complete information during registration and to update such information as necessary. You agree not to share your account credentials with any third party.

Service Packages and Pricing

Care Bridge offers second opinion services through defined service packages. Current packages and pricing are displayed on our platform. We reserve the right to modify pricing with reasonable notice to users.

An annual Access Membership fee is required for platform access. The Access Membership is non-refundable once activated. Service package fees are separate from the Access Membership.

Payment and Refund Policy

Payments are processed through Stripe, our secure payment provider. No protected health information is included in any payment metadata — only opaque order identifiers, generic service descriptions, amounts, and currency.

Refund eligibility depends on the stage of service delivery. If a case has not yet been assigned to a physician, a full refund may be issued. Once a physician has begun review, partial refunds may be considered on a case-by-case basis. Completed and delivered reports are not eligible for refund. Contact our support team for refund inquiries.

Physician-Patient Relationship

The second opinion provided through Care Bridge constitutes an independent advisory review. It does not replace the care of your local treating physician and does not constitute an ongoing physician-patient treatment relationship.

Care Bridge physicians provide medication considerations for review by your treating physician. These considerations are advisory in nature and should be discussed with your local physician, who has the full context of your medical history and can make appropriate treatment decisions.

Intellectual Property

The Care Bridge platform, including its design, features, content, and technology, is owned by Care Bridge Medical, Inc. and protected by intellectual property laws. You may not copy, modify, distribute, or create derivative works based on our platform without our prior written consent.

Second opinion reports delivered to you are for your personal use and may be shared with your treating physicians. You retain ownership of your medical records and health information that you upload to the platform.

Limitation of Liability

To the maximum extent permitted by applicable law, Care Bridge Medical, Inc. shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or relating to your use of our services.

Care Bridge's total aggregate liability for any claims arising from or related to these Terms or your use of our services shall not exceed the total fees paid by you to Care Bridge in the twelve (12) months preceding the event giving rise to the claim.

Nothing in these Terms limits liability for gross negligence, willful misconduct, or any liability that cannot be excluded or limited by applicable law.

Governing Law

These Terms are governed by the applicable law identified in Care Bridge's final customer terms and operating records. Any dispute process will be handled according to the applicable terms presented at the time of service.

Termination

You may terminate your account at any time by contacting our support team. We may suspend or terminate your access to our services if you violate these Terms, engage in fraudulent activity, or if continued service would be unlawful.

Upon termination, your right to use the platform ceases immediately. We will retain your medical records for the period required by applicable healthcare regulations.

Changes to These Terms

We may update these Terms from time to time. We will notify you of material changes by posting the updated Terms on our platform and updating the effective date. Your continued use of Care Bridge after such changes constitutes your acceptance of the revised Terms. If you do not agree with the updated Terms, please discontinue use of our services.
